Booking An Appointment
Online Booking
You are now able to book your appointments online using askmyGP. Once you have created an online request, a Medical Staff Member will be in contact with you as soon as possible. To start, simply click here to go to askmyGP or the ‘Appointments’ button at the top of this page (Desktop).
Please note, you will need to have an account to use askmyGP. If you are unsure of how to use this service and would like a walkthrough, please watch the video below.
Telephone Booking
Whilst we would ask that our patients utilise askmyGP to book their appointments online, we understand that not everyone may be able to do so. If you do not have access to a smartphone, PC or the internet you are welcome to call the Practice, where a member of our Reception Team will be able to assist you with your request.
Registration & Appointment Types*
Home Visits
If you are too ill to attend the surgery and you require a home visit, please phone the surgery before 10.30am. You may be called by a clinician first to determine the urgency of your visit.
Please note, we cannot guarantee your request will be accepted and it may be forwarded to the Ambulance Service or another care provider.

Registration
New here? We would be happy to welcome you to the Practice.
To register as a patient you will need to fill out our Registration Form . This is all online based, but paper copies are available from the practice. You may be asked to provide photo identification, if you are registering for online services. Following our receipt of the registration form, you will be registered with us within 2 working days. You will not need to notify your previous practice of your reregistration.
If you are already a patient and wish to register for online services, amend your details, or register with the Patient Participation Group (PPG), please see our Medical Forms.
Telephone Consultations
Based on your request, the clinician will decide if a Face to Face or Telephone Consultation would be more appropriate.
If you would prefer a telephone consultation, please note this within your request.
Video Consultations
Based on your request, the clinician will decide if a Face to Face or Telephone Consultation would be more appropriate.
If you would prefer to have a video consultation, please note this within your request.
Additionally, at times the clinician you see will ask if they can record your consultation. This is for their ongoing clinical practice and contributes towards their appraisal and/or training. You will be asked for your consent to this prior to any video recording and it is completely optional.

Fit Notes*
Less Than 7 Days
If you have been unwell for 7 days or less you don’t need to see a Doctor. You can complete a Self-Certification Form yourself. Many employers have their own self-certification forms.
However some employers insist on a Doctor’s note regardless. A Doctors note under these circumstances involves a charge of £30.
More Than 7 Days
If you have been unwell for more than 7 days you will need a doctor’s certificate. These certificates are called ‘Fitness To Work’ Certificates.
Please contact the practice to request a certificate. We will let you know when you can collect your Fit Note, or we may contact you to arrange an appointment.
*Note: A Fit Note can also be referred to as a Sick Note, Medical Form, Med 3s, or Doctor's Note.

Out of Hours
Enhanced Access Appointments*
The practice, as part of Sleaford Network PCN, offers pre bookable appointments between the hours of 6.30pm to 8pm (weekday) and 9am to 5pm (Saturday). These are held with a variety of clinicians and are bookable via Reception.
These appointments are routine and can be used for routine GP or Nurse appointments. When booking an appointment you may need to consent to a consulting clinician that may not be your own GP viewing and updating your health record.
This service is available to any patient registered locally, bookable via their own practice.

Urgent Care Unit*
The practice runs the Urgent Care Unit Monday to Friday 6.30pm -8.00pm and Weekend/Bank Holidays 8.00am -6pm. The service is for newly presenting conditions (within the last 48hrs) of minor illness such as chest infection, ear infections and UTI’s.
This service is available to all people, regardless of their registered GP Practice.
Minor Injury*
This service is available weekday 8.30am to 8.00pm and Weekend/Bank Holidays 8.00am to 6pm. The injury must have occurred within 48 hours and we must be the first port of call.
This service is available to all people, regardless of their registered GP Practice.
After 6.30pm
If you require assistance after 6.30pm please contact the NHS Out of Hours Service by dialling 111, where you will be directed to the appropriate service.
In an emergency dial 999.
*Note: These services will be closed on Christmas Day, Boxing Day and Easter Sunday.

Reception staff roles are multifaceted including booking an appointment, clinical coding, and dealing with general admin enquiries. The Reception Department is a dedicated team of staff who work in rotation. We can be contacted via phone or askmyGP, and look forward to helping you with your enquires.
