Coronavirus Care Plan – Notice to Patients

We here at Sleaford Medical Group want to reassure our patients who have been identified as vulnerable or are shielding during the pandemic, that they will receive contact from us.

We are contacting those identified to ensure that they are adequately supported at this time and, as per NHS England guidance, we have been advised to offer a coronavirus care plan.

This care plan is just a brief summary of a patient’s health record and identifies a patient’s wishes and preferences should they become unable to voice them. Completion of a care plan is entirely voluntary and can be declined.

If you have not yet received contact from the practice and are within the vulnerable or shielding group, you will be contacted over the coming weeks.
